What is DHI?

DHI (Direct Hair Implantation) is a hair-transplant implantation approach that commonly combines FUE graft extraction with placement using an implanter device, often called a Choi-type implanter pen. Follicular units are extracted individually from the donor area, prepared and loaded into the implanter, then placed into the recipient area with controlled depth, angle and direction.

How DHI Works — Step by Step

1. Consultation & Hairline Planning

Hair-loss pattern, donor density, scalp condition, facial proportions and future hair-loss risk are assessed. The surgeon designs a conservative, natural-looking hairline.

2. Donor Area Preparation

The donor region is prepared and local anaesthesia is administered. Depending on the case, shaving may be complete, partial or limited.

3. FUE Graft Extraction

Follicular units are individually scored and extracted with an FUE punch from the safe donor region. Extraction should be distributed carefully to avoid visible over-harvesting.

4. Graft Sorting & Preservation

Extracted grafts are inspected, counted and kept hydrated. Single-, double- and multi-hair follicular units may be sorted according to the implantation plan.

5. Loading the Implanter Pen

A trained team carefully loads each follicular unit into an appropriately sized implanter needle. Excessive manipulation or compression of the follicle should be avoided.

6. Direct Implantation

The surgeon uses the implanter to create/enter the recipient site and place the graft in a controlled motion. Depending on technique and case, some recipient sites may also be pre-made rather than every graft being implanted without prior incisions.

7. Angle, Direction & Density

Hairline grafts are positioned to mimic natural emergence angles and direction. Density is planned according to donor supply, recipient blood supply, hair caliber and long-term strategy.

8. Final Inspection & Aftercare

The recipient and donor areas are checked and individualized washing, medication, activity and follow-up instructions are provided.

What is a DHI / Choi Implanter Pen?

An implanter pen is a spring-loaded or manually controlled device with a hollow needle. A follicular unit is placed inside the needle, and the device assists controlled placement into the recipient area. Different needle diameters may be selected according to graft size and hair characteristics.

Potential Advantages of DHI

  • Precise control of graft placement, including angle, direction and depth in experienced hands.
  • Useful for detailed hairline work and implantation between some existing hairs when clinically appropriate.
  • Can reduce repeated handling during placement when the loading and implantation workflow is efficient.
  • May allow limited-shave or recipient no-shave approaches in selected patients.
  • Implanter sizes can be matched to different follicular-unit dimensions.

Limitations — What DHI Cannot Guarantee

  • DHI does not create additional donor grafts and cannot overcome a weak donor area.
  • It does not guarantee higher density, higher graft survival, less bleeding, faster healing or a scar-free result.
  • Very dense implantation can still compromise recipient tissue if performed beyond safe limits.
  • Results depend heavily on surgeon planning, extraction quality, graft handling, team efficiency and patient biology.
  • DHI can be slower and more labor-intensive in some large sessions because grafts must be loaded into implanters.

DHI vs Conventional FUE/Sapphire FUE

The main distinction is usually the implantation workflow. In conventional FUE, grafts are extracted by FUE and recipient sites may be created first with steel or sapphire blades before graft placement with forceps or other implantation tools. In DHI, grafts are commonly placed using implanter pens. These approaches overlap: FUE describes extraction, while sapphire and DHI mainly describe recipient-site/implantation choices. No single method is automatically superior for every patient.

Who May Be a Good Candidate?

  • Patients with an adequate, stable donor area.
  • Patients requiring refined hairline or frontal-zone implantation.
  • Selected patients who prefer limited shaving.
  • Some patients needing implantation around existing hairs, provided the surgeon judges it safe.
  • Patients with realistic expectations and a long-term hair-loss management plan.

Who Needs Extra Caution?

  • Very poor or miniaturized donor areas.
  • Active scalp infection or uncontrolled inflammatory scalp disease.
  • Unrealistic expectations or requests for unsafe density/very low hairlines.
  • Significant uncontrolled medical conditions or bleeding risks.
  • Extensive progressive hair loss without an adequate lifetime donor strategy.

Recovery & Growth Timeline

Mild swelling, redness, tenderness and crusting can occur during the early postoperative period. Transplanted hair shafts commonly shed during the first several weeks. New growth usually develops gradually over subsequent months. Cosmetic improvement becomes progressively more noticeable, while maturation may continue for around 12 months and sometimes longer, particularly in crown areas. Timelines vary between patients.

Risks & Possible Complications

Potential risks include bleeding, swelling, pain, infection, folliculitis, crusting, itching, temporary numbness, shock loss, donor over-harvesting, visible dot scars, poor or uneven growth, unnatural angle/direction, pitting or cobblestoning, asymmetry and dissatisfaction. Rare complications are also possible.

Aftercare Essentials

  • Do not rub, scratch or pick the recipient area.
  • Follow the clinic's exact washing schedule and technique.
  • Take only prescribed/recommended medicines as directed by the treating clinician.
  • Avoid activities that may traumatize or heavily contaminate the grafts during the early healing period.
  • Attend scheduled follow-ups and contact the treating team for unusual pain, pus, fever, persistent bleeding or other concerning symptoms.
