FUT Hair Transplant — Step-by-Step Guide
Follicular Unit Transplantation (FUT), often called the strip method, is a hair-restoration technique in which a narrow strip of hair-bearing scalp is surgically removed from the safe donor area, usually at the back of the head. The strip is microscopically dissected into individual follicular-unit grafts, which are then implanted into the recipient area. The donor wound is closed with sutures or staples, leaving a linear scar that is usually concealed by surrounding hair.
Step 1: Consultation & Assessment
The surgeon assesses the pattern and grade of hair loss, donor density and scalp laxity, hair calibre, medical history, previous surgery, expectations, and the risk of future hair loss. FUT is selected only when it fits the patient’s donor characteristics and long-term plan.
Step 2: Hairline & Graft Planning
A natural hairline and recipient-area plan are designed. Grafts are allocated according to the frontal zone, mid-scalp and crown, while preserving enough donor reserve for possible future hair loss.
Step 3: Donor Strip Planning
A narrow ellipse is marked within the permanent donor zone. Strip width and length are planned according to scalp laxity, donor density and the required graft number. Harvesting outside the stable donor zone should be avoided.
Step 4: Local Anaesthesia
The donor area is cleaned and anaesthetised. The recipient area is also anaesthetised before site creation and implantation.
Step 5: Strip Harvesting
The planned strip of scalp is carefully removed. Surgical technique aims to minimise follicular transection and protect surrounding donor hair.
Step 6: Donor Closure
The donor wound is closed, commonly with sutures or staples. A trichophytic closure may be considered in suitable cases to help hair grow through the scar, but a linear scar remains an expected feature of FUT.
Step 7: Microscopic Graft Dissection
The harvested strip is divided under magnification into naturally occurring follicular units. Grafts are trimmed carefully and sorted, while remaining hydrated in an appropriate holding solution.
Step 8: Recipient-Site Creation
Tiny recipient sites are created according to the planned density, direction, angle and depth. Hairline sites are designed to reproduce a soft, natural transition.
Step 9: Graft Implantation
Single-hair follicular units are commonly used along the leading hairline, with larger units placed behind them for visual density. Grafts are inserted gently to minimise trauma.
Step 10: Final Check & Dressing
The surgeon checks symmetry, graft placement and donor closure. The patient receives written post-operative instructions and medication when clinically indicated.
Step 11: Early Recovery
Temporary soreness, tightness, swelling, redness, crusting, itching or numbness can occur. Sutures or staples are removed according to the surgeon’s protocol. Patients should avoid trauma and strenuous activity during early healing.
Step 12: Shedding & Growth
Transplanted hair shafts commonly shed during the first several weeks. New growth develops gradually over subsequent months, with maturation continuing for about a year or longer; crown results may mature more slowly.
Potential Advantages
FUT can yield a large number of grafts in one session without creating thousands of extraction punches across the donor area. It may be useful in selected patients requiring substantial graft numbers and can preserve donor-area surface for possible future FUE harvesting. Grafts are dissected under magnification, allowing careful preparation of follicular units.
Limitations & Risks
FUT requires surgical strip removal and produces a permanent linear donor scar. Recovery can involve more tightness or discomfort than FUE, and very short hairstyles may reveal the scar. Other possible complications include bleeding, infection, wound problems, widened or hypertrophic scarring, temporary numbness, shock loss, folliculitis, poor graft growth and an unnatural result. Scar appearance varies between patients.
FUT vs FUE
FUT removes follicular units from a surgically excised donor strip, whereas FUE extracts individual follicular units directly with small punches. Neither technique is automatically superior for every patient. The choice depends on donor density, scalp laxity, hairstyle preference, graft requirement, previous procedures, scarring tendency and long-term donor management.
Important Note
The number of grafts should be individualized. It should not be promised solely on the basis of a photograph or a fixed package. Safe donor limits, scalp characteristics and future hair-loss progression must guide surgical planning.

مرحلہ 5: Strip Harvesting
Marked Donor Strip کو Surgical Technique سے احتیاط کے ساتھ نکالا جاتا ہے تاکہ Follicular Damage اور Transection کم سے کم ہو۔
مرحلہ 6: Donor Closure
Strip نکالنے کے بعد Wound کو Sutures یا Staples سے بند کیا جاتا ہے۔ منتخب مریضوں میں Trichophytic Closure استعمال کی جا سکتی ہے، لیکن FUT میں Linear Scar پھر بھی متوقع ہوتا ہے۔
مرحلہ 7: Microscopic Graft Dissection

مرحلہ 8: Recipient Sites بنانا
Recipient Area میں Hair Direction، Angle، Depth اور مطلوبہ Density کے مطابق باریک Sites بنائے جاتے ہیں۔ Hairline کو Soft اور Natural Appearance دینے کے لیے خاص Planning کی جاتی ہے۔
مرحلہ 9: Graft Implantation
Hairline کے سامنے عموماً Single-Hair Grafts اور ان کے پیچھے Larger Follicular Units لگائے جاتے ہیں تاکہ Natural Transition اور Visual Density حاصل ہو۔
مرحلہ 10: Final Check اور Post-Op Instructions
Surgeon Graft Placement، Symmetry اور Donor Closure چیک کرتا ہے۔ مریض کو Post-Operative Care اور ضرورت کے مطابق Medicines کی ہدایات دی جاتی ہیں۔
مرحلہ 11: ابتدائی Recovery
ابتدائی دنوں میں Soreness، Tightness، Swelling، Redness، Crusting، Itching یا Temporary Numbness ہو سکتی ہے۔ Sutures/Staples Surgeon کے Protocol کے مطابق Remove کیے جاتے ہیں۔
مرحلہ 12: Shedding اور Growth
ابتدائی ہفتوں میں Transplanted Hair Shafts کا جھڑنا عام ہو سکتا ہے۔ نئے بال آنے والے مہینوں میں آہستہ آہستہ بڑھتے ہیں اور Final Maturation تقریباً ایک سال یا اس سے زیادہ لے سکتی ہے۔ Crown Area کو زیادہ وقت لگ سکتا ہے۔
FUT کے ممکنہ فوائد
منتخب مریضوں میں FUT ایک Session میں بڑی تعداد میں Grafts حاصل کرنے میں مدد دے سکتا ہے اور پورے Donor Area میں ہزاروں Punch Extractions نہیں بناتا۔ Microscopic Dissection کے ذریعے Follicular Units کو احتیاط سے Prepare کیا جا سکتا ہے۔ بعض Long-Term Donor Plans میں FUT کے بعد مستقبل میں FUE کی گنجائش بھی محفوظ رہ سکتی ہے۔
نقصانات اور ممکنہ Risks
FUT میں Surgical Strip Removal ہوتی ہے اور ایک Permanent Linear Donor Scar بنتا ہے۔ FUE کے مقابلے میں Tightness یا Discomfort زیادہ ہو سکتا ہے اور بہت Short Hairstyle میں Scar نظر آ سکتا ہے۔ دیگر Risks میں Bleeding، Infection، Wound Problems، Widened/Hypertrophic Scar، Temporary Numbness، Shock Loss، Folliculitis، Poor Graft Growth اور Unnatural Result شامل ہو سکتے ہیں۔
FUT اور FUE میں فرق
FUT میں Follicular Units ایک Surgically Removed Strip سے حاصل کیے جاتے ہیں، جبکہ FUE میں Individual Follicular Units کو Small Punches کے ذریعے براہِ راست Donor Area سے Extract کیا جاتا ہے۔ ہر مریض کے لیے ایک ہی Technique بہترین نہیں ہوتی۔ فیصلہ Donor Density، Scalp Laxity، Hairstyle Preference، Required Grafts، Previous Procedures، Scarring Tendency اور Long-Term Donor Management کے مطابق کیا جاتا ہے۔
